TERMINAL PERFORMANCE OBJECTIVE

At the end of 306 hrs. Related Learning Experience, the students will be able to;

Acquire the holistic understanding of the human person as a bio-psycho-cultural being focusing on the concept of health and illness as it is related to the care of the mother and child in the varied settings.

Demonstrate the competencies in the following key areas of responsibility such as safe and quality nursing care, communication, collaboration and teamwork, health education, legal responsibility, ethico-moral responsibility, personal and professional development, quality improvement, research, management of sources and environment and records management

ENABLING OBJECTIVES

Describe the health care delivery system and the role of the nurse in it. Demonstrate ethic-moral, legal responsibilities in the care of the individual family & community. Demonstrate the beginning skills in the provision of independent & collaborative nursing functions. Relate the stages of growth & development in the care of the clients.

Demonstrate beginning skills in the preparation of healthy & therapeutic diets in varied clients. Explain the dynamics of the disease process caused by the microbes & parasites and the environment. POLICIES ON PROMOTION
Students must successfully pass all courses required in order to be promoted to the next level. The standing of the student in each subject is based upon the results of prelim, midterm, prefinal and final exams. As well as the daily quizzes, oral exam and demonstrations of skills throughout the semester.

A semester is divided into four terms or rating periods as follows:

For professional subjects like NCM 101 & CHN which has Classroom LEC & RLE units. The computation of final grade is the combination of final grade of classroom LEC and RLE grade depending on the weighted unit for LEC and RLE.

For Ex.: NCM 102(8 units): LEC 5 units ; RLE 6 units NCM (lec) final grade = 82% x 5 units = 410 NCM (rle) final grade = 83% x 6 units = 498 908 11 units NCM 101 average Grade = 82%

FAILURES
At the end of the semester, a student who fails in any 2 minor subjects will not be promoted to the next term, however, he can be allowed to take up other subjects of which the subjects he/ she failed are not pre-requisites. At the end of the semester, a student failing in 1 major subject may be promoted to the next level, provided that he/she repeats that subject & can only take minor subjects.

A student failing in 2 major subjects will not be promoted to the next level but be allowed to repeat those subjects in the next school year or term it is offered.
A student failing 2 times in the same major subject will be dismissed from the course. A student who fails in any nursing foundation subjects shall repeat that subject and is not eligible to enroll in other advance nursing subjects until he/ she passes that failed subject.

Students with an average rating from 3.1 to 3.4 may be given a REMOVAL

EXAMINATION.

Those whose average rating ranges from 3.5 to 5.0 will be given a failing mark of 5.0 for the subject area.

The highest grade that a clinical instructor can give to a student who passes the removal examination, whether she/ he gets 75 or higher is only 3.0. REMOVAL EXAMINATION is given only ONCE per semester.

The giving of incomplete grade (INC) is discouraged. Students who fail to take the term exam for prelim, midterm & pre-finals with valid reason will be given 10 days to take the exam after exam week, otherwise will be given grade of 50 which is 60% transmutated score. For Example; Quiz(40%) = 90% *90 x .40 = 36 T. Exam(60%) = 50% *50 x .60 = 30 Prelim Grade 66%-Failed However, students who fail to take the FINAL EXAMINATION may be given an INCOMPLETE GRADES (INC).

No student will be allowed to enroll the next semester without completing his/her grade in any subject.
He/ she is not eligible for the Level III.

Refers to the required number of hours based on the number of units in consonance to professional subjects, that the student has to practice work in the clinical area for the entire semester.

Payback Duty or Clinical Extension: Refers to the number of make up days to be rendered by a particular student based on deficiencies in the required number of RLE hours in a semester, in the affiliating agencies

DROPPED & FAILED STATUS: A student is considered dropped or/and failed in RLE when:
The number of hours lost due to absences (excused or unexcused), is more than 20% of the prescribed no. of hours in practice work or RLE before the prelim examination, the student shall be dropped from the subject.

The absences of more than 20% of the total number of hours prescribed in RLE is incurred after the prelim or succeeding terms, the student will automatically be considered as failed.

Excused absences are paid back with the same number of hours or days that the student was absent in the clinical area. These are absences incurred under the following situations or conditions.

A. Illness an excuse slip with a medical certificate duly signed by the attending physician countersigned by the school physician and duly signed by the Level Coordinator and Dean. A.1. A student who was hospitalized due to illness will incur a payback equivalent to the number of days confined.

A.2. A student who was absent due to illness with medical certificate only will incur a payback twice the number of days absent. B. Death (3 days excused absence) immediate members of the family (Father, Mother, Brother, Sister, Husband, Wife, Child only) supported by pertinent documents.

Force Majeure typhoons, floods, earthquake, fire or any circumstances which makes it impossible to report on duty. A document to support such incident should be presented to the clinical instructor duly signed by the Level coordinator and Dean. Absence due to official function/ activity with pertinent document(s) presented duly signed by the moderator and the Dean.

When the student fails to report with the complete paraphernalia in the clinical area.

When the student fails to submit a clearance or Evaluation card from previous area of assignment duly signed by the previous clinical instructor.

Students are expected to report to the clinical area on time. A maximum of 15 minutes shall be allowed for TARDINESS.

ALL MEDICAL CERTIFICATION and LETTERS requesting for excused absences must be submitted to the Clinical Instructor (CI) with a copy furnished for the office, or may be submitted by a representative in case the student is not physically fit to submit the medical certificate personally.

Students shall only be given 2 working days from the last date of absence to submit the medical certificate or letter of excuse to aforementioned entities.

Failure to submit the medical certificate or explanation letter related to such absence to the assigned Clinical instructor within the given period, will automatically forfeit the excuse of the student.

In the clinical area are paid back 2 times the number of absences the student has incurred. Unexcused absences are situations/conditions such as the following:

A. When the student fails to report to the right area of assignment due to negligence in checking the rotation schedule. B. When the student fails to notify the clinical instructor about request for excused absence on the scheduled day of rotation. C. Attending personal or family functions such as reunions, weddings or similar forms of gatherings.

It should be settled by the student on or before payback duty is completed. Receipt of payment from the school cashier should be submitted to the level coordinator. Student will not be cleared unless payback fee is settled even if required number of days is rendered.

1. The Special Examination shall be known as: a. Removal Examination b. Late Term Examination 2. Definition; a. Removal Examination- a final exam given to students with semestral grade averaging at 3.1 to 3.4.

b. Late Term Examination- an exam given to students who failed to take the Term exam on the set examination week.
The student will be given the grace period of ten (10) ten days from the last of the term examination to take the special examination. Failure to do so will result to a term examination grade of 5.0 which covers 60% of the term grade.

However, if the period of recovering from illness or surgery warrants a longer period of rest as deemed necessary by the school physician, the student will take his/her special examination on the first day he will be fitted to report in school.
A student is only allowed to take the special exam after payment of special exam fee. The Receipt will serve as a permit to take the special exam.

A. When sick but to present a medical certificate duly signed or countersigned by school physician for cases in which the family physician issued the said certificate. B. When financially constraint but to let parent or guardian visit the office to affirm such reason. Instruct parent or guardian to obtain promissory status duly approved by the Finance Officer.

The following are valid reasons for students to qualify for a special exam or term exam:

C. When an immediate family member specifically the father, mother, sister, brother, husband, wife, or child died provided that a document will be presented as proof of the their demise.
A student can only avail one special exam in a subject per semester. Consequent taking up of special exam in the same subject is very much discouraged.

However, if and when there are reasons or evidences for the instructor with the level coordinator confirming that student demonstrates habitual requests for special exam. the parent or guardian shall be called by the DSA upon forwarding of a written report signed by the subject instructor and noted by the coordinator and DEAN.

COLLEGE HONORS: It is an award given by the college for exemplary academic performance. College Honors is composed of two categories as follows: First Honors Category Second Honors Category

Each category is defined based on criteria set in which the student should satisfactorily meet. An Honor Card which is one form of certificate of recognition shall be awarded to the qualified student issued by the Registrar with the DEANs approval, stipulating the category of College Honors and the General Average obtained for the semester/summer.

The recipients of the College Honors shall be awarded during the Recognition Program held every end of the second semester of each school year. However, the Registrar shall post on bulletin boards the List of College Honors for each semester & summer within two weeks from the last day of classes.

FIRST HONORS Category Criteria: The candidate must carry the regular academic load for the semester. Must earn a General Average of 88 or 2.0 in all academic subjects, and that the student shall not have a grade lower than 85% or 2.25 in any prescribed subject. Should have no adverse records on file with the College and Dean of Discipline office.

SECOND HONORS Category Criteria The candidate must carry the regular academic load for the semester. Must earn a general Average of 85% to 87.99% in all academic subjects, and that the student shall not have a grade lower than 80% or 2.7 in any prescribed subject. Should have no adverse records on file with the College and Dean of Discipline office.

I have no money to pay upcoming term examination, what can I do?

SOLUTION

Approach your Subject Teacher and inform him/her about your concern BEFORE THE TERM EXAM to assist you. Write a Promisory Note duly signed by the student and the guardian/parent and addressed to:

Frequently Asked Questions


I missed the term exam because I failed to obtain a ledger because I have no money to PAY, can I just attend class for the next term? I will just get the exam that I missed at the END of the SEMESTER.

NO. you will not be allowed to attend class for the next term NOT UNLESS you take the EXAM within 10 days after the scheduled exam.

After 10 days, FAILURE to get the LONG EXAM Prelim, Midterm, Prefinals- will automatically obtain a grade of 5.0

Each students must provide at least two Hijab to ensure cleanliness. Embroided name of the hospital should be at the neckline part of the Hijab which is visible to read either at the Right or Left. All Hijab are available at the BRENT-COOP. In the hospital clinical area, for students in RLE, hijab is permitted in the Adult ward, Pedia ward, De luxe, & Private rooms, ER, & OPD (provided that they are not handling communicable cases). All white hijab should be without sequins or any accessories in replace of the nurses cap. Students who are assigned in Special areas are not allowed to wear their hijab. Hence, it is permitted once the students is in the conference room.

In the event that students insist of downing a hijab in the special area compromising infection control, the students are advised to sign a waiver to be reassigned to other areas or in the community.
