Management of sick Young infant from birth up to 2 months

DIARRHEA

ASSESSMENT

A. Check for very severe disease and local bacterial infection


Is the infant having Count the breaths per minute.50 breaths ___ difficulty of feeding? Repeat if 60 breaths or more. __ Fast? Has the infant had Look for severe chest indrawing. Fever (temperature 37.5 or feels hot) convulsions? Low body temperature (below 35.5C) Look at the umbilicus. Is it red or draining pus? Look for skin pustules. Look at the young infants movements. Does the infant move on his own? Less than normal? * Does the infant move only when stimulated? * Does the infant not move at all?

Very Severe Disease!!

TREATMENT
Give first dose of intramascular antibiotic. *Ampicillin 1.3 ml & Gentamicin 3.2ml Treat to prevent low blood sugar * If the infant is not able to breastfeed but is able to swallow (Give 20-50ml of sugar water) How? Dissolve 4 level teaspoons Of sugar (20 grams) in a 200-ml cup of clean water. Refer urgently to the hospital. Advice mother on how to keep the infant warm on the way to the hospital. * Provide skin to skin contact * Keep the young infant clothed or covered as much as possible all the time. Dress the young infant with extra clothing, including a hat, gloves and socks; wrap him/her in a dry cloth and cover with a blanket.

B. Then Check for Jaundice


If jaundice present, when did jaundice first appear? Look for jaundice (yellow eyes or skin) Look at the young infants palms and soles. Are they yellow?

C. Does the young infant have diarrhea?

Yes___ No ___

7 For how long? ___ days Look at the young infants general Is there any blood in stools? condition. * Does the infant move only when stimulated? * Does the infant does not move at all? * Is the infant restless or irritable? Look for sunken eyes. Pinch the skin of the abdomen. Does it go back: Very slowly (>2 seconds)? Slowly?

CLASSIFICATION

Severe Dehydration!!

TREATMENT
If infant has a VERY SEVERE DISEASE, SEVERE JAUNDICE, or DYSENTERY: > Refer URGENTLY to hospital, with mother giving frequent sips of ORS on the way > Advise mother to continue breastfeeding > Advice mother how to keep the young infant warm on the way to the hospital. * Provide skin to skin contact * Keep the young infant clothed or covered as much as possible all the time. Dress the young infant with extra clothing, including a hat, gloves and socks; wrap him/her in a dry cloth and cover with a blanket.

D. Then check for feeding problem or low weight for age


Is there any difficulty in feeding? YES ___ NO ___ Is the infant breastfed? YES ___ NO ___ If yes, how many times in 24 hours? ____ times Look for ulcers or white patches in the mouth. (thrush) Does the infant usually receive any other foods or drinks? YES ___ NO ___ If yes, how often? ___ What do you use to feed the child? _______________

Determine weight for age. LOW ____ NOT LOW ____

E. Check the young infants Immunization status:

Feeding advice
> Continue breastfeeding frequently as often as long as the infant wants > Do not give other foods or fluids

Care for development advice (up to 4 months)


A. Play > Provide ways for your child to see, hear, feel and move B. Communicate > Look into your childs eyes and smile at him/her. > Breastfeeding is a good time to talk to your child and get a conversations going with sounds or gestures.
