Disciplines : Human Reliability Analysis
Human Reliability Analysis can be define as the use of systems engineering and behavioral science methods in order to render a complete description of the human contribution to risk and to identify ways to reduce that risk (identifying ways of reducing system vulnerability to human error impact). This is achieved through three processes: Identifying what errors can occur - Human Error Identification

Deciding how likely the errors are to occur Human Error Quantification
Enhancing human reliability by reducing this error likelihood - Human Error Reduction

Disciplines : Human Reliability Analysis
Wherever a not wanted event occurs (design, acquisition, construction, operation and maintenance) its causes could be associated to the behavior (conduct) or condition:
Associated at Behavior (conduct ) 94%

What is associated at behavior could be classified as follows:

Associated at Condition (procedure error)

6%
CAN NOT DO IT
59%

DOES NOT KNOW HOW TO DO IT

35%

Why people do not do what they suppose to do?

Disciplines : Human Reliability Analysis
These reasons of a not wanted event occurrence (can not do it, does not know how to do it, does not want to do it) are directly related with factors that cause low performance levels and affect operational effectiveness (Motivation, Knowledge, Resources, Skills and Expertise):
CAN NOT DO IT 59% DOES NOT KNOW HOW TO DO IT 35%

Intrinsic Objective: Improve Human Reliability to lower accidentality level and occurrence of not wanted events.

DOES NOT WANT 6% TO DO IT

Studying these factor allows to learn and deal with not wanted events and its causes.

Due to narrow correlation among factors (each factor is affected by the other two) their separated study and treatment produce partial results and sometimes in longer terms. Therefore an integrated approach is proposed
