COMPARATIVE EVALUATION OF THE EFFECT OF DIFFERENT ROOT CANAL PREPARATION TECHNIQUES IN PRIMARY TEETH USING CBCT

NEED FOR STUDY:

Bacteria and their by- products plays an essential role in the initiation
and perpetuation of pulpal and periapical diseases .

The primary objective of cleaning and shaping the root canal system are removing soft and hard tissue containing bacteria ,providing a path for irrigants to the apical third ,supplying space for medicaments and subsequent obturation, retaining the integrity of radicular structure .thus success of pulpectomy depends on elimination of irritants by means of cleaning and shaping the root canal

Primary objectives of canal preparation thus involves removal of


organic substrate from the canal system by chemo-mechanical preparation and the three dimensional shaping of the root canal system into a continuously tapering preparation while maintaining the original outline and form of the canal.

Many studies have been done to evaluate dimensional changes


in permanent teeth after instrumentation with manual, rotary and ultrasonic files. Evidence suggests that rotary instruments cut more dentin and may there for reduce torsional loads; however more aggressive cutting could produce increased canal transportation. With the advent of CT scans such volumetric analysis have become more accurate and easy.

This technology can evaluate not only cross-sections of roots,


but also three dimensional shapes of canals at higher resolution.

There are however, limited studies to assess the changes that can
occur in primary teeth post instrumentation with different endodontic techniques.

Hence, this study has been undertaken to compare the changes taking place in primary teeth after root canal preparation using three different techniques; the rotary Ni-Ti, ultrasonic instrumentation and conventional method.

RESEARCH QUESTIONS:

Are the effects of Ni-Ti rotary and ultrasonic files comparable to


conventional files when used for root canal preparation in primary teeth?

NULL HYPOTHESIS:

Ni-Ti rotary files and ultrasonic files are comparable to conventional


files in their shaping ability when used in root canals of primary teeth.

OBJECTIVES OF THE STUDY:

To evaluate the shaping efficiency of rotary Ni-Ti, ultrasonic and


conventional method of canal preparation in primary teeth with the help of CBCT. To compare the shaping efficiency of Ni- Ti, ultrasonic with conventional method of canal preparation in primary teeth.

SOURCE OF DATA:

75 freshly extracted, primary human teeth will be

collected

The teeth collected will be cleansed of visible blood and debris


and maintained in a hydrated state. CBCT will be done to assess the root canals,before proceeding for the canal preparation. Now, the samples will be divided into three groups, according to the root canal preparation techniques employed as follows.

Group A: 25 teeth to be prepared with conventional K files

Group B: 25 teeth to be prepared with REVO-S


(rotary) system.

Group C: 25 teeth to be prepared with ultrasonic


instrumentation.

Inclusion criteria: The teeth that will be used for the study should have a minimum root length of 8 mm. or at least two third roots remaining.

The teeth may be extracted for the following reasons :

Infected primary teeth with considerable bone loss. Over retained primary teeth with altered root resorption pattern. Primary molars with one root resorbed considerably more than the other, due to the altered path of eruption of its successor. Infected primary teeth with chronic recurrent infection.

Exclusion criteria:
Teeth with more than two third of root resorbed. Fractured teeth Endodontically treated teeth Roots with more external and internal resorption Teeth containing restoration
