Assessment of Student s performance by SV Consists of 19 attributes (ITC- E) A joint assessment by SV and UTP Supervisor during 2nd visit The form should be returned to UTP Supervisor or SIIU by HC SV or sent it directly to SIIU (in a sealed envelope)

INTERNSHIP VISIT

1ST VISIT - week 5 - 8

courtesy visit by lecturer (could be from different programme). Form ITC- B : to be filled up by the UTP supervisor, Host supervisor and student. To prepare training schedule (submit together with ITC-B form to UTP Supervisor) and logbook

2nd VISIT week 25 28

Oral presentation and evaluation by Host Supervisor and UTP supervisor. To complete logbook Form ITC-D, ITC-E & Summary of weekly scores (to the complete weeks) must be submitted to UTP Supervisor.
