Contractor Safety Orientation


This presentation is to be used as an overview of the safety rules and policies at the Arkansas Kraft Division facility. It does not cover all the safety requirements detailed in our Safety Policy Manual. All contractors and contractor employees must be trained on and understand all the requirements of our Safety Policy Manual before beginning work at Arkansas Kraft Division.

Lockout

It is the responsibility of each person inspecting or working on equipment to follow the Lockout procedure as outlined in the Arkansas Kraft Division Lockout Policy. Personnel who violate these rules are subject to disciplinary action. Lockout is required when: Any employee (or contractor) is required to remove or bypass a guard or other safety device. Any employee (or contractor) is required to place any part of his body into the mechanism of a piece of equipment or path of hazardous energy/materials while installing, constructing, erecting, adjusting, inspecting, cleaning, operating or maintaining the process.

Lockout
Lockout Procedures: Equipment will only be de-energized and locked out by personnel authorized by Arkansas Kraft Division who have been trained in the Lockout procedure and who are familiar with the specific procedures for the equipment. Contractor employees may lockout on the energy source or on the group lockout device only after the Authorized Arkansas Kraft Division employee has secured the equipment, verified a successful lockout and placed a personal or department lock on the device. Personal locks must have only 1 key. Contractor employee personal locks must have the name of the employee, the name of the contractor and a Danger tag on the lock or on a tag attached with the lock. The name of the owner must be legible at all times. Silver Master locks are not to be used for Lockout.

Lockout

Lockout Procedures: When the Group Lock Out Procedure is utilized, personnel working on the locked out equipment are required to: Review the lock out isolation list to ensure it is for the piece of equipment they will be working on. Review the lock out isolation list to ensure that it is complete. Place their personal lock on the lock ring or lockbox prior to performing assigned work and remove their personal lock after completing assigned work. Note: never place a personal lock on the lock ring or lockbox until the lockout isolation list is complete and posted and the department lock is applied

Lockout

Lockout Procedures: Each individual working on a piece of equipment that is locked out must place his/her personal lock on the appropriate lockout device. Individuals must maintain possession of the key to their personal lock. No one other than the individual that placed the lock on the lockout device may remove the lock. Removal of a personal lock by anyone other than the person that placed the lock may result in disciplinary action. Contractors may use a satellite lock box to protect their employees. The contractor supervisor or designee will place a Contractor lock and tag on the Arkansas Kraft Division lockout device. The contractor will then place the key to this lock in his lockbox. Contractor employees will then place their personal locks on the Contractor lockbox.

Lockout

This orientation provides a brief overview of the AKD lockout requirements. All Contractors and contractor employees must be trained on the requirements of the AKD Lockout Policy and provide to AKD proof of training and understanding of the requirements of the Lockout Policy before performing work at Arkansas Kraft Division.

Confined Space

The Arkansas Kraft Division Confined Space Policy is designed to control and protect personnel from Confined Space hazards and to establish guidelines for entry into Confined Spaces. This policy prohibits unauthorized entry, identifies and evaluates potential hazards, and establishes procedures and practices for safe entry. This includes atmospheric monitoring, requires an attendant for all permit spaces during entry operations, establishes procedures to summon rescuers and a system for preparing, issuing, using and canceling entry permits.

Confined Space

A Confined Space is defined as an area which (1) has adequate size and configuration for employee entry, (2) has limited means of access or egress, and (3) is not designed for continuous employee occupancy. A Permit-Required Confined Space is a Confined Space as defined above, and presents or has the potential for hazards related to atmospheric conditions (toxic, flammable, asphyxiating), engulfment, configuration or any other recognized serious hazard.

Confined Space

Signs shall be posted near Confined Spaces to notify individuals what hazards might be present and that only Authorized Entrants may enter the Confined Spaces. Signs shall read, DANGER, Permit Required Confined Space DO NOT ENTER.

Confined Space

An individual is considered to have entered the Confined Space when any part of the individual's body breaks the plane of the Confined Space's opening. Only individuals who are trained and knowledgeable in the hazards they may face, are able to recognize signs or symptoms of exposure and understand the consequences of exposure to hazards may enter confined spaces. Entrants must know how to use any needed equipment, communicate with Attendants as necessary, alert Attendants when a warning symptom or other hazardous condition arises and exit as quickly as possible when ordered to or alerted.

Confined Space

Attendants are individuals who are trained in and familiar with the hazards of Confined Spaces, aware of behavioral effects of potential exposures, know to maintain a continuous count of entrants, remain outside the space until relieved, communicate with entrants as necessary to monitor their status and when to order exit. They must be properly equipped and know how to summon assistance, prevent unauthorized entry and perform non-entry rescues. They must not perform other duties that interfere with their primary duty to monitor and protect the safety of Authorized Entrants. Attendants must wear a green vest while serving as an attendant.

Confined Space

An authorized AKD employee shall properly shutdown and isolate the confined space. An authorized AKD employee will complete all tasks required by the permit for entry using the procedures detailed in the Arkansas Kraft Division Confined Space Policy. The completed permit will be posted at the entry point after all lockout and atmospheric testing has been completed. Contractor employees will lockout on the appropriate lockout device before entering a confined space. The atmosphere inside the space will be monitored continuously when personnel are in the space.

Confined Space

No individual shall enter any confined space until it has been properly prepared and permitted for entry using the procedures detailed in the Arkansas Kraft Division Confined Space Policy. No individual shall enter any confined space without an attendant posted at the entry point. Entrants must ensure their name and time of entry is on the entry list when they enter and exit time is on the list when they exit the space. Entrants must place their personal lock on the lockout device before they enter the space and remove their personal lock when they finish working in the space.

Confined Space

Entrants must wear a harness and retrieval line when entering a confined space unless they create a hazard to the entrant. Contractors shall supply their employees equipment as required by the job (gas monitors, safety lines/harnesses, other PPE). Authorized Entrants shall exit the Confined Space, unless it is physically impossible to do so without assistance, when: the Attendant orders evacuation; an atmospheric monitor alarm is activated; the Authorized Entrants perceive that they are in danger.

Confined Space

All Contractors and contractor employees must be trained on the AKD Confined Space Policy and provide to AKD proof of training and understanding of the requirements of the Confined Space Entry Policy before performing work at Arkansas Kraft Division.

Personal Protective Equipment

Personal Protective Equipment (PPE) required at AKD ANSI Z 41 1991 approved safety shoes are mandatory. Hard hats are to be worn at all times by contractors. Hearing protection must be worn in areas above 80 decibels. Appropriate, approved eye protection must be worn at all times. Tinted or dark glasses will not be worn inside buildings except when used for welding . Gloves must be worn by all personnel when handling rough, sharpedged, abrasive material, or where work subjects the hands to lacerations, punctures, burns or bruises.

Personal Protective Equipment

Fall Protection Personal Fall protection is required when working on elevated surfaces 4 feet high or more without protective barrier. Personal fall protection includes a full body harness with shock absorbent type lanyard. Anchor point for fall protection must hold at least 5000 pounds. Employees required to use fall protection must be trained in the use, inspection and maintenance of the equipment used.

Personal Protective Equipment

Dress requirements Contact lenses are not allowed in the mill. Jewelry including watches, rings, earrings, necklaces, and bracelets are not to be worn in the mill areas. Loose fitting clothing must not be worn Hair restraint must be worn if the hair is long and touching the shoulders. Shirts with four (4) inch sleeves down past the shoulders and that cover the entire mid-section of the body is required. Full-length trousers without excessive length and flare are required.

This orientation provides a brief overview of the Arkansas Kraft Division Personal Protective Equipment requirements. All Contractors and contractor employees must be trained on the AKD Personal Protective Equipment Policy and provide to AKD proof of training and understanding of the requirements of the Personal Protective Equipment Policy before performing work at Arkansas Kraft Division.

Emergency Response

The purpose of this Emergency Response Policy is to provide an effective Emergency Action Plan that identifies planned responses for emergencies which have a reasonable probability for occurrence at Arkansas Kraft Division. For the purpose of this plan, an emergency is any occurrence or situation with the potential to cause loss of life or extensive injuries to personnel, extensive property damage, or damage to the environment. An emergency may include, but is not limited to a major fire, explosion, serious injury, or chemical spill. This Policy applies to all individuals working at Arkansas Kraft Division if an emergency occurs.

Emergency Response

The basic step to take in any emergency at AKD is to notify the Guard. The Guard is designated to be the main communications coordinator in any emergency that can occur in the mill; consequently, the response to any emergency will effectively start by notifying the Guard. To report any emergency call 9111. When reporting an emergency be prepared to provide the following information: Name and telephone extension Nature of the emergency; i.e., fire, spill, injury, bomb threat, etc. Location of emergency situation Details of situation

Emergency Alarms There are 7 different types of alarms throughout Arkansas Kraft Division: 1. Emergency Response Alarm (Alert for Fire, Rescue or Hazmat Team) 2. Severe Weather Alarm 3. Hydrogen Sulfide Alarms (local to affected area) 4. Boiler Alarm 5. Evacuation Alarm 6. Fire System Alarms (Local to areas) 7. Natural Gas Leak alarm

Emergency Response

Hydrogen Sulfide Detection and Alarms Hydrogen sulfide is generated in the pulping processes at Arkansas Kraft Division. This highly hazardous chemical is monitored in the mill by gas detection monitors. It smells like rotten eggs. Detection of hydrogen sulfide will activate local alarms. Red flashing light Evacuate area, be alert for changing environmental conditions Red Flashing Light and Horn -- Hazardous concentration exits, Evacuate area immediately and notify your supervisor.

Emergency Response

Recovery Boiler Alarm If a problem develops with the boiler, a horn will sound at the boiler. When the boiler alarm sounds, all outside contractors, and visitors should immediately leave the area via a safe route. Contractors should report to the evacuation area and sign out at the gate. Do not use an elevator.

Natural Gas Leak Alarm If a Natural Gas leak is detected, the mill alarm will sound 4 30 second blasts. Contractors should evacuate to the designated evacuation area

Emergency Response

Any employee, contractor, visitor, or vendor that is not in their assigned work area, when an evacuation occurs and cannot reach their assigned assembly point, should proceed to the nearest accessible assembly point and report their presence to the person in charge. This person should remain with this group throughout the duration of the evacuation.

General Safety Rules

The only safe source of drinking water is a bottled water or drinking fountain; other sources should not be used. Possession of or drinking of alcoholic beverages is strictly prohibited on Company premises. Possession or use of drugs for other than medical reasons is forbidden on Company premises. Everyone is expected to keep their equipment and work area clean. Smoking is allowed only in designated areas. Check with the AKD supervisor for smoking areas. Smokeless tobacco must have container with lid. Do not spit anywhere except in the container. Keep the container with you at all times.

General Safety Rules

Injury Reporting Procedure All injuries, regardless of severity must be reported to supervisor. All first aid injuries require written report to be turned in to Guard, Nurse or Safety Office Contractor must have first aid kit. Minor first aid to be administered by contractor. Nurse phone number is 9230, Guard phone number is 9350. Report serious injuries by calling 9111. Med-Tech will be called to assist.

Safety Flagging and Signs


Danger flagging. Red danger flagging indicates immediate danger and special precautions are necessary. Danger flagging must not be crossed by unauthorized personnel. Caution flagging. Yellow caution flagging is used to warn against potential hazards or to caution against unsafe practices. Personnel may enter the flagged areas if they have a compelling reason to enter the area; however, they should avoid entering the area whenever possible. Barricade tags. All flagging must have barricade tags attached identifying who placed the flagging, why it is in place, when it was placed and how long it will be used.

Cell phones may be used only by supervisors and only for company business. Vehicle speed limit is 15 mph in the mill. All tools must be identifiable to the contractor, color code, engraving, marking, etc. Tools must be inspected before use, damaged tools must be repaired or replaced. Lost or missing tools should be reported to the guard.

Compressed Air Air hose pressure must be regulated or pressure relieved below 30 PSI Inspect hoses for damaged or loose clamps Use Safety clips Never use an air hose with compressed air against your body or clothes or that of another person. Do not use mill air for human supplied air High pressure hoses Use your own or have them made up by maintenance

Extension Cords and Welding Leads Must have valid inspection program Must have identification labeling Must have ground fault circuit interrupters on all extension cords and Electrical hand tools Extension Cords and Welding Leads must not be run through doors or holes in walls, keep off floor Extension cords and welding leads should be run overhead when possible and must not pose a tripping hazard.

General Safety Rules

Flammable liquids stored inside a building must be in an approved flammable-liquid storage cabinet. Only authorized personnel are allowed to enter the electric centers. Smoking, tobacco, food and drink are prohibited in the electrical centers.
Personnel are expected to obey all signs, tags and flagging which are posted to warn of potential or existing hazards. Only properly trained and authorized personnel can drive and operate Mobile Equipment and Cranes. Requirements for operating such equipment including pickup trucks, dump trucks, Forklift and Clamp Trucks, House Cranes, Gantry Cranes, Mobile Cranes, and Manlifts can be found in the AKD Mobile Equipment Policy.

Ladders and Scaffolding Only ladders and scaffolds that meet OSHA requirements and that are in good condition will be used. Only fiberglass ladders are to be used around electrical energy sources. Ladders must extend 3 feet above landing and be tied off. Ladders must be placed at 4 to 1 ratio at base. Scaffolding must be erected and inspected by qualified and competent person. Working platform for scaffolding must be covered. If scaffolding is over 20 feet high, must be tied off If over 10 feet high, must use fall arrest system. Ladder must be used to get on and off scaffolding.

General Safety Rules

Any unknown insulation material is to be treated as asbestoscontaining matter. Asbestos containing material is marked with purple paint. If you suspect material is asbestos, do not handle or disturb it. Contact AKD personnel. Persons in contact with chemicals must wear proper protective equipment and clothing. If, for any reason, it is necessary to remove handrails, U-drain covers, etc., make sure that a temporary means of protection is provided.

General Safety Rules

Operate only equipment that you have been trained and authorized to operate. Observe all traffic signs, speed limits, and parking regulations. The speed limit in the mill for all traffic is 15 mph. Go in and out of the plant by regular roadways and walkways. Do not take shortcuts or go through other department posted areas, such as motor control centers, recycle fiber, shipping, etc. Cross railroad tracks at regular roadways only, after taking time to "Stop, Look, and Listen."

General Safety Rules

Horseplay of any kind is absolutely forbidden at the Arkansas Kraft Division. Do not pass under, over, or between railcars at the facility. When discarding boards, always remove protruding nails or bend them down. Protective blinds must be used around welders to prevent other employees from receiving arc burns.

Firearms and other weapons are forbidden on Arkansas Kraft Division property. Know the emergency alarms, evacuation routes and assembly points for your specific work area. Be familiar with emergency safety equipment located in your work area. All individuals working at Arkansas Kraft Division must adhere to all other established safety rules/policies developed by the Arkansas Kraft Division.

General Safety Rules

Golf carts, mules and similar transportation must be approved by maintenance superintendent or designee. Golf carts, mules and similar transportation must be equipped with operable front and rear lights, horn, brakes and brake lights. Portable generators shall not be connected to any AKD electrical device or circuit without prior approval of AKD electrical superintendent or designee. Lost or missing tools must be reported to security or personnel.

Permits

Confined Space Entry


Required for entry into any confined space at AKD. Completed by authorized AKD personnel. AKD personnel will isolate and lock out space. Contractors must lockout on lockout device provided by AKD. Trained attendant must be provided by contractor. Atmosphere must be tested before entry and monitored while in space. Sign in and out with time on entry roster Full body harness and lifeline required. See AKD Confined Space Policy for details about the permit.

Permits
Hot Work Permit Required for any work that contributes a direct source of ignition such as cutting, welding, grinding, heat-treating, powder actuated fasteners, and other similar activities that can result in a flame or spark being generated. Completed by authorized AKD personnel. Contractor must furnish trained firewatch. Firewatch must have and know how to use firefighting equipment. Firewatch must remain for 1 hour after completion of work. Contractor must notify Production Supervisor to monitor area for additional 3 hours.

Permits
Hot Work Permit

Completed Hot Work Permit must be turned in to Safety Office. Call 9111 to report Fire. Do not use fire hose stations to clean up with. Fire hose used for fire watch must be turned off, drained and returned to rack at completion of firewatch.

Permits

Permits required Excavation Permit - Required for any manmade cut, cavity, trench, or depression in the earths surface formed by earth removal. Excavations over four (4) feet in depth are considered confined spaces. Completed by Authorized AKD employee. See AKD Excavation Policy for details about the permit. Line Break Permit Required prior to breaking a line or opening process equipment that contains or could contain hazardous contents. Completed by Authorized AKD employee. See AKD Line Break Policy for details about the permit.

Contractor Access

Hazard Awareness

Processes and equipment at Arkansas Kraft Division may pose hazards to personnel that are not properly trained to work around them. Contractor employees are not allowed in areas where they are not trained and authorized to access. Be aware of the signs, labels and markings in the mill that warn of potential hazards.

Hazard Awareness

We have several alarms in the Pulp/Steam & Recovery areas that everyone that works in or travels through the area must be aware of. These include: Hydrogen Sulfide alarms at the Lime Kiln and Evaporators These alarms will sound if we have a release of hydrogen sulfide gas in the areas. If these alarms sound, evacuate the area immediately and notify a supervisor or the guard. Do not return to the area until the alarm is no longer sounding.

Hazard Awareness

We have several alarms in the Pulp/Steam & Recovery areas that everyone that works in or travels through the area must be aware of. These include: Pulp mill blow heat alarm This alarm sounds 3 minutes before we blow a digester in the Pulp mill. When this alarm sounds, everyone must move at least 30 feet away from the blow heat system until the blow is finished. It takes approximately 13 minutes to blow a digester. Signs are posted at this location. If you will be working in this area within 30 feet of the blow heat recovery system, notify the Pulp mill operators before beginning work. They will make arrangements to ensure your safety while in the area.

Hazard Awareness

We have several alarms in the Pulp/Steam & Recovery areas that everyone that works in or travels through the area must be aware of. These include: Recovery Boiler rapid drain alarm This alarm is located at the Recovery Boiler and will sound if we have an emergency with the boiler. If this alarm sounds, evacuate the area immediately and stay clear until the emergency is resolved and you are advised that you may return.

Hazard Awareness

All containers of chemicals and hazardous substances must be labeled and indicate the hazards and precautions to be taken if handling or exposed to the substances. This includes tanks, vessels, cans, totes, etc. Material Safety Data Sheets (MSDS) are available upon request for any chemical in the facility. Contractor must have prior approval to bring chemicals into mill and provide MSDS for chemicals Observe all warnings and report any spills to your supervisor.

Hazard Awareness

Vehicular and heavy equipment traffic is constant throughout the facility. Pedestrians must travel in designated walkways and cross roadways at designated crossings.
Powered industrial truck traffic is heavy in the recycle fiber and shipping areas. Do not enter these areas unless authorized to do so. Inform the drivers that you are in the area. When working in these areas always listen and watch for truck traffic. Be cautious when rounding corners and walking through doorways. Always be alert for moving vehicles throughout the facility.

Contractor Safety Requirements

In addition to the requirements above, contractors and contractor employees must be trained on the requirements of the AKD Contractor Safety Policy.

Contractor Safety Requirements

Contractors must provide written verification that all their employees have been trained on and understand all Arkansas Kraft Division safety requirements and policies listed in this orientation before the employees begin work at the Arkansas Kraft Division facility.
