Class I Wells
Extensive permitting requirements No-migration demonstration for hazardous wells Continuous annulus monitoring (except municipal) Internal MIT every year (every five years for most Class I-non-hazardous) Frequent reporting
Clase II
Class II Wells
Dispose of salt water produced with oil or natural gas (II-D) Inject fluids for enhanced recovery (II-R)
Store liquid hydrocarbons (II-H)
Class II Wells
May have multi-well area permits
Existing enhanced recovery wells (i.e., drilled before State program approved) may be authorized by rule for the life of the well Must demonstrate mechanical integrity at least every 5 years Monthly monitoring of injection pressure, flow rate, and volume
WELLHEAD
Injection pressure gauge Injected fluid Valves
Annular access
Area of Review
Goal: Identify area where injection will raise injection zone pressure enough to endanger aquifers
- Define Area of Review/Zone of Endangering Influence
- Regulations allow mile fixed radius or calculation. Identify all wells of public record or otherwise known to applicant that penetrate injection zone-construction/and or plugging records
Repair any deficient construction/plugging and/or monitor fluid level. Identify water wells, springs, landowners, faults and fractures, etc. of public record or otherwise known to applicant
Confining Zone
Must demonstrate there is at least one low permeability zone, separating injection zone from USDWs
Must be free of faults/fractures that can allow fluid movement in Area of Review Must have sufficient areal extent to protect USDWs.
Mechanical Integrity
Initial Cementing Records, Cement Bond Log Pressure test tubing/casing annulus every 5 years and whenever tubing pulled or if mechanical integrity lost
Pressure annulus to 10% above max. permitted injection pressure, monitor for 1 hour Loss of >5% pressure is failure.
Financial Responsibility
Third Party Cost Estimate for Plugging Well(s) Financial Mechanism to cover cost estimate. Periodic updates as required by Region 2. Financial statements updated annually.

Class IV Wells
Used to dispose of hazardous or radioactive waste into or above a formation which contains a USDW within 1/4 mile of the well
Prohibited
One exception: wells that reinject into same formation treated ground water pursuant to approved CERCLA or RCRA clean-ups
Class V Wells
Class V: All injection wells that do not meet the definitions of Classes I, II, III, or IV
Most are shallow and low-tech Most inject into or above USDWs

Orden Administrativa
