To determine the adverse reactions, effects of drug errors and determine factors affecting medication administration in the palliative units. To determine factors or barriers that leads to inadequacy of treatment and management.

Exploring Factors that Influence Informal Caregiving in Medication Management for Home Hospice Patients
This study shows different factors that may impede the delivery of medications. One of which is the caregivers limitation in managing medications such as: a. negative emotional states which are grief and fatigue, that may impair decision making, b. cognitive and physical impairments, specially older spouses or partners, c. low literacy may impede learning and understanding of prescription instructions. Another is that employment, housekeeping, and other dependents may compete with care giving. Negative emotional state that may lead to patients refusal of medication and complex medication entail complicated instruction which increases care giving demands. And also poor interpersonal relationship among multiple caregivers may adversely influence medication management.

Palliative care in cancer: An overview


Results: Palliative care is now an accredited professional discipline. In general, palliative care models can be provided in 3 different settings: domiciliary, inpatientor outpatient/consultative. Cancer palliative care is a multidisciplinary task, shared by informal and formal caregivers. Cancer pain relief in palliative care is part of a comprehensive pattern of care. Due to the lack of adequate studies, limited impact on patients quality of life has been demonstrated, although patients subjected to palliative care have the potential to have a good death. Conclusion: Palliative care on a cancer patient treatment has positive effects on patients and on the other people involved. Thus, it is important to adopt palliative care since the moment of diagnosis.

Cancer pain in Jordan: prevalence and adequacy of treatment This study showed that 73.3% of the patients had pain, severe enough to merit rating on the BPI. The mean of the worst pain scores in the past 24 hours was high at 7.5 out of 10 . 31 % of patients with pain had not been treated for their pain, and the mean Pain Management Index score was -1.20. In conclusion, these cancer patients were experiencing high levels of pain, which was undertreated. This means that despite the availability of treatment options for cancer pain management , pain is still undertreated .

Barriers to cancer pain management: a review of empirical research The most significant patient related barriers were patient reluctance to report pain and adherence to treatment recommendations. The most prominent physician related barriers were insufficient physicians knowledge about cancer pain management, inadequate patterns of pain assessment, and inadequate opioid prescription. Therefore, patient- and physician-related barriers have a remarkable negative influence on the quality of cancer pain management.

Roles and systems for routine medication administration to prevent medication errors in hospital-based, acute care settings: a systematic review A number of factors were identified as contributing to medication errors for routine medication administration, including mathematical skills, knowledge of medications, quality of the prescription, length of experience, shift patterns, workload and staffing levels, medication delivery systems, single-nurse administration, policies and procedures. Acute care settings bring additional concerns for routine medication administration, particularly increased pressure on staff, increased number of drugs, polypharmacy, increasingly similar-sounding drug names, increased throughput of patients, and wider ranges of illnesses and treatments.

IN TOTALITY ,WE WOULD LIKE TO RECOMMEND THE FF: comprehensiveness of medication administration to reduce drug errors since it is the palliation that must be promoted and by doing so, we promote the quality of life of patients as well. Educational efforts to provide caregivers : increase in self-confidence, knowledge on unified policies and guidelines regarding pain management. Simplifying medications to simpler regimen instructions and using pill management system for easy access of medication. improving symptom assessment by using assessment tools to guide the health care professionals in order to minimize under treatment.
