Clinic Visit

The patient visits the Health Center/clinic to avail the services there offered by the facility primarily for consultation. He also asks for consultation on the ailments he currently has. Question: What should the PHN do?

Answer: There are certain procedure that are performed at the clinic visits. They are as followed:

1.) Registration/Admission 2.) Waiting Time 3.) Triaging 4.) Clinical Evaluation 5.) Laboratory and other diagnostic examinations 6.) Referral System 7.) Prescription/ Dispensing 8.) Health Education

Blood Pressure Measurement


After admitting the client, the PHN should obtain the clients blood pressure. These are the proper steps in obtaining BP, which are: I. Preparatory Phase II. Applying the BP cuff and stethoscope III. Obtaining the BP readying by auscultation IV. Recording BP and other guidelines

Home Visit
The PHN goes to an area to conduct an interview and gather data to a certain family Purpose of Home Visit: *To give nursing care to the sick, to a post partum mother and her newborn child. *To assess the living condition of the patient and his family and their health practices in order to provide the appropriate health teaching

To give health teachings on prevention and control of diseases. Establish a close relationship between the health agencies and the public for health promotions To make use of the inter-referral system and to promote the utilization of community service.

Principles involved in preparing for a home visit


When visiting a home; it is necessary to assemble all records of the client and lists of names to be visited. A home visit must:

Must have a purpose or objective Have a plan so that all available information about the family are used. Planning and delivery of care should involve the individual and family The plan should be flexible

Guidelines regarding the frequency of visits

There is no definite rule to be followed on the frequency of home visits. They may vary according to the needs of the client

Physical, psychological and the educational needs of the individual and family. Acceptance of services of the family; their interest and willingness to cooperate. Evaluation of the past services given to a family The ability of the client and his family to recognize their own needs, their knowledge of available sources and their ability to make use of their resources for their benefits

Steps on conducting home visits


Greet the patient and introduce yourself State the purpose of the visit Observe the patient and determine the health needs Put the bag in a convenient place then proceed to perform bag technique Perform Nursing care needed and provide health teachings Record all important data and observations. Make an appointment for next visit
