There is pale violet discoloration of the anterior wall of

Bimanual examination of gravid uterus shows extreme
softening of the cervix.
MacDonalds Sign
On account of the softening of the isthmus, the fundus of
the uterus is anteflex, so by bimanual examination, there is an
easy approximation of the fundus and the cervix.

Hearing of the Fetal Heart

Sounds (Mayors Sign)
The fetal heart sound is compared to that of the
ticking of the watch under the pillow.
Location is at the anterior abdominal wall and
is dependent upon the presentation and position
of the fetus.

Outlining of the
Fetal Parts
By palpation, we can determine the
head, neck, arm, back and buttocks. If
these parts could be outlined, the
pregnancy is sure.
However, it may be confused with
irregular ovarian cyst.

Laboratory Test
for Pregnancy
1.Pregnancy Slide Test

a) Principle An agglutination-inhibition reaction is used to

demonstrate the hormone HCG which is excreted into the
urine during pregnancy.
b) Reagents (1) Pregnancy Slide Test antiserum (2)
Pregnancy Slide Test antigen
c) Procedure Place 1 drop of urine, then 1 drop of PST
antiserum in one of the circles on the test slide and mix
thoroughly with a stirring rod. Add 1 drop of PST antigen 2 .
Mix well with a stirring rod. Carefully agitate the slide with
a circular motion ensuring that the fluid revolves all over
the circle. Read the result after 2min.
d) Interpretation of result No agglutination means positive
e) Sensitivity HCG concentration of 1.5-2.4 IU/ml urine
f) Remarks Fresh urine preferably in the morning.

Signs of Pregnancy in
the Dead
Presence of Ovum
or Fetus
Examination will reveal
product of conception
together with the
placenta, amniotic fluid,
and membrane.

Findings on the
Uterus Itself
Examination will reveal
thickening, increase in
size and capacity of the
uterus. The mark of
placental
attachment
may be seen.

Corpus Luteum

Examination may reve

a well-developed corpu
luteum with a certa
size, however, it ma
gradually retrogress.

Duration of Pregnancy
Average duration is 270 280 days from the onset of last menstruation.
It is hard to ascertain the exact date of fertilization.

Abnormally Prolonged Gestation

In all cases where the gestation much over 300 days is alleged, confirmatory
evidence should be expected in the exceptional weight and size of the child.

Minimum Period of Gestation Compatible with Viability of

the Child
A child born at 180 days of gestation may live.
Before that period, may exhibit signs of life but cannot continue to live, owing
the want of development of the breathing muscles and breathing center.

Methods of Estimating the

Date of Expected Delivery
o

From the date of the first day of the last menstruation, add
seven days and count three months backwards.

Count forward nine calendar months from the date of the first
day of the last menstruation and add one week.

Count forty weeks or ten lunar months from the date of the first
day of the last menstruation.

Pregnancy

Imaginary pregnancy usually observed in

women nearing menopause or in
younger women who are very desirous of
having children.
Patients will present:
All the subjective symptoms.
Increase in abdominal size due to abnormal fat
deposition or tympanism.
Abnormalities with the menses.
Fetal movements which are in reality muscle

Diagnosis of Fetal
Death

1. Repeated examinations of the uterus show that the size remains stationary even after
a lapse of a number of weeks or months.
2. Endocrine test for pregnancy is negative on more than one occasion.
3. Cessation of fetal movements after they have been felt.
4. Absence of fetal heart sounds after a repeated and prolonged examination.

Later months
of pregnancy

5. Positive signs of fetal death as shown by the palpation of softened macerated

fetal head, with bones freely moveable on each other and the scalp hanging
over a loose sac.
6. The breast cease to enlarge and become soft and flabby.
