1. Is the cover sheet provided with proper project

title and proper names/signature of officials
with designations of approving/concurring
officials?
2. Is the Index of sheets/drawings complete with
proper sheet number and set numbers?

3. LOCATION MAP/VICINITY MAP

Is the location of proposed bridge with respect to existing road,
road section, towns, etc. indicated?
Are the important landmarks/existing structures indicated?
Are the contours indicated?
Are the location of boreholes indicated?
Are the direction of stream flow indicated?
Are the stationings indicated?
Are the bench marks indicated?
Is the azimuth of bridge longitudinal axis indicated?
Is the directional sign/informatory sign indicated?
Is the location of private and public structures/utilities that maybe
affected by the proposed project indicated?
Is the location of existing bridge and possible detours, if any,
indicated?
Is the Title Block with proper signatories indicated?

4. GENERAL NOTES
. Are the codes and specifications indicated?
. Is the Highway Loading indicated?
. Is the Design Stresses indicated?
. Are the codes and specifications indicated?
. Is the material specification indicated?
. Is the construction procedure/sequence indicated?
. Is the special provision/information, if any, indicated?
. Is the notes for foundation (bored piles, R.C. piles, spread
footing, etc.) indicated?
. Is the Title Block with proper signatories indicated?

5. SUMMARY OF QUANTITIES IN TABULATED FORM

. Are the Item Numbers correct?
. Are the Description of Work (or Item Description) correct?
. Are the Units of Measurement correct?
. Are the Quantities for Abutment, Piers and Superstructures
separately presented?
. Are the total for all items involved indicated?
. Are the original quantities and new (revised) quantities
separately indicated in case of revised plans/As-staked or AsBuilt plans?
. Is the Title Block with proper signatories indicated?

6. GENERAL PLAN AND ELEVATION

Is the span/length indicated?
Is the Bridge Type indicated?
Is the stationing (Piers, Back of Backwalls) indicated?
Is the directional sign/information sign indicated?
Is the Ground Profile showing the elevation of ordinary water
level (OWL), maximum flood level and design flood level
indicated?
Is the type of foundation (bored piles, R.C. piles, Steel H-piles,
spread footing, etc.) indicated?
Is the finish grade elevation indicated?
Are the position/spacing of railing indicated?
Are the hydraulic data indicated?
Is the location and side elevation of drain grouted riprap and
other bridge protection works indicated?
Are the position/spacing of drain indicated?
Is the clear roadway and sidewalk width indicated?
Is the title block with proper signatories indicated?

7. FOUNDATION PLAN AND LAYOUT

. Are the boring logs and location indicated?
. Are the elevation and stationings of SPT indicated?
. Are the N-values indicated?
. Are the description and soil classification of boring logs
indicated?
. Is the plan of footing showing size of footing and arrangement
of piles indicated?
. Is the Title Block with proper signatories indicated?

8. DETAILS OF SUPERSTRUCTURES
8.1 F0R REINFORCED CONCRETE DECK GIRDER
Is the elevation showing dimensions, girders reinforcement, railing
and rail post reinforcement indicated?
Is the plan showing dimensions, slab reinforcement, diaphragm
reinforcements, location of drains, railing and railing post
distances, bearing details indicated?
Is the typical roadway, cross-section half showing dimensions and
half showing slab reinforcement, sidewalk reinforcement and
railing reinforcements indicated?
Is the crown slopes of superelevation indicated?
Is the camber diagram indicated?
Is the bar bending diagram and bar bending schedule indicated?
Is the estimate of quantities indicated?
Is the typical section of girders, deck slab diaphragm showing
dimensions and rebars indicated?

8.2 FOR PRESTRESSED CONCRETE GIRDER

Is there notes for prestressed concrete girder?
Is there separate detail of girders showing position of prestressing
cables and end anchorages?
Is the dowels and diaphragms location indicated?
Is the details of prestressing cables and end anchorages
indicated?
Is the required jacking force and net prestressing force indicated?
Is the girder section details, one at the end block, one at
intermediate and one showing girder dimensions indicated?
Is the estimate of quantities indicated?
Is the Title Block with proper signatories indicated?

8.3 FOR STEEL TRUSS SUPERSTRUCTURE

Is the general view showing elevation, plan, top and bottom view
with dimensions, number of joints and dimensions indicated?
Is the summary of stresses in tabulated form indicated?
Is the list of drawings for details of truss members indicated?
Is the camber diagram indicated?
Is the general notes (specification, deadload, liveload concrete
slab, reinforcing steel, structural steel, bolts/rivets, and member
designations indicated) indicated?
Is the details of expansion joints indicated?
Is the estimate of quantities indicated?
Is the Title Block with proper signatories indicated?

9. MISCELLANEOUS DETAILS
Is the details of drain indicated?
Is the details of railing and sidewalk indicated?
Is the details of expansion and fixed bearings indicated?
Is the details of elastomeric bearing pad indicated?
Is the details of expansion dam/joints indicated?

10. DETAILS OF PIERS

. Is the plan, front and side elevation half-showing dimensions
and half-showing reinforcements indicated?

. Is the position of bearings, centerline of joints, centerline of pier
and eccentricity, if any, indicated?

Is the 4.88m vertical clearance for highway/underpass /tunnel

Is the L/800 superstructure deflection for rural areas complied?
Is the L/1000 superstructure deflection for urban areas

Is the design discharge Q=50 years return period complied?
Is the 4.00m minimum roadway width (1 lane) complied?
