Patient: Albert Ross DOB: 01/03/79
Patient presents with a restricted flow of urine and pain while urinating. He
also feels discomfort of the bladder since he has trouble expelling waste.
Patient has been experiencing these symptoms for approximately four days.

Patient provided a clean catch urine simple for analysis. I ruled out major
diseases of the kidney since only benign casts were presents in the urine.
Tests revealed no signs of infection anywhere in the urinary system. Further
examination of the patient revealed blockage of the urethra due to kidney
stones.

I recommended thet Mr. Ross increase his fluid intake to pass the stones. I
also advised him that dietary changes might help prevent recurrence.

I referred him to a nutritional specialist for further imformation. Mr. Ross will
return in two weeks for a follow up.
El paciente presenta un flujo restringido de orina y dolor al orinar. Tambin
siente incomodidad de la vejiga ya que tiene problemas para expulsar los
desechos. El paciente ha estado experimentando estos sntomas durante
aproximadamente cuatro das.

El paciente proporcion una orina de captura limpia simple para el anlisis.

Descart las principales enfermedades del rin ya que slo se presentaron
moldes benignos en la orina. Las pruebas no revelaron signos de infeccin en
ninguna parte del sistema urinario. Un examen posterior del paciente revel
bloqueo de la uretra debido a clculos renales.

Le recomend que el Sr. Ross aumentara su ingesta de lquidos para pasar las
piedras. Tambin le aconsej que los cambios en la dieta podran ayudar a
prevenir la recurrencia.

Le mencion a un especialista en nutricin para una mayor informacin. El Sr.

Ross regresar en dos semanas para un seguimiento.
