CT kontras merupakan cara yang akurat, non invasive untuk
mengidentifikasi perdarahan aktif pada pasien fraktur pelvis
Lokasi ekstravasasi kontras dapat terlihat dalam CT scan sesuai
baiknya dengan lokasi perdarahan yang terlihat dalam
angiografi pada pasien dengan pendarahan fraktur pelvis
Dapat melokalisasi arteri yang terjadi perdarahan sehingga
prosedur transcatheter arterial embolization dapat dilakukan
lebih dini, sehingga menurunkan morbiditas dan mortalitas.
On CT images, contrast material extravasation located anterior to the
sacroiliac joint that is associated with sacroiliac joint disruption or within
the iliacus muscle hematoma with associated fractures of the ilium is
highly suggestive of injury to the iliolumbar artery
(a) CT scan shows a fracture of the right inferior pubic ramus with extravasation of
contrast material (arrow) within a hematoma in the ischiorectal fossa.
(b) Superselective angiogram obtained with a microcatheter shows active
bleeding (solid arrow) from the internal pudendal artery. Note the normal

Pelvic arterial bleeding due to injury of visceral branches in patients with pelvic fracture is
quite rare. Extravasation of contrast material in the bladder wall can be seen on contrast-
enhanced CT scans when the vesical arteries are injured

Merupakan treatment of choice pada pasien
dengan perdarahan arteri akibat fraktur pelvis
Pasien tidak stabil hemodinamik meskipun telah
diresusitasi urgent arteriography dan embolization
direkomendasikan
Pasien yang menjalani CT kontras, angiographic
embolization dilakukan untuk mengontrol
perdarahan arteri yang mengancam jiwa ketika
ekstravasasi kontras terlihat pada CT scan.
Efektivitas (Success rate) dalam mengontrol
perdarahan dan mengurangi kebutuhan tranfusi
mencapai 85-100%
TAE harus dilakukan sesegera mungkin embolisasi efektif harus
tercapai sebelum sistemik koagulopati berat dan multiple organ failure
terjadi.
