El aumento de la eficacia,
seguridad y simplicidad del
TARGA como opciones de
terapia de rescate, la mejora
en el aumento de la esperanza
de vida del paciente infectado
por VIH, ha vuelto a plantear
la discusin sobre un inicio
ms temprano del TARGA.
Introduccin
El momento ideal de inicio de
TARGA que proporcione el
mximo beneficio aun no esta
claro.
En los ltimos aos han
tratado de establecer pautas a
partir de estimaciones riesgo
de infecciones oportunistas,
evolucin a SIDA y muerte.
Introduccin
Recientes estudios de algunas condiciones clnicos no
relacionadas con el SIDA, deben de ser considerados
en la definicin riesgo de progresin de la infeccin.
Personas asintomticas con CD4 altos, la replicacin
viral y la activacin inmune crnica se asociaron con
desarrollo de enfermedades que tradicionalmente no se
han relacionado con la infeccin del VIH.
No hay estudios clnicos aleatorizados que al momento
respondan a la duda.
Los estudio observacionales, podran sobrestimar el
beneficio de la intervencin.

Recomendaciones
El inicio del TAR debe basarse en tres elementos:
los sntomas, el nmero de CD4 y la CVP.
En caso de infeccin sintomtica (eventos B o C)
se recomienda iniciar el TAR en todos los casos
(nivel A).
Si la infeccin es asintomtica el inicio del TAR
se basa en el nmero de CD4, la CVP o en
determinadas comorbilidades o caractersticas del
paciente

1. Si los linfocitos CD4 < 350 clulas/ l se recomienda el
TAR (nivel B).
2. Si los linfocitos CD4 estn entre 350 y 500 clulas/ l se
recomienda el TAR, aunque se puede diferir su inicio en
determinadas situaciones, como la presencia de un
recuento de CD4 estable, CVP baja o poca predisposicin
del paciente (nivel B).
3. Si el nmero de linfocitos CD4 es mayor de 500 clulas/l
se recomienda diferir el TAR en general; sin embargo,
debe ser considerado en los pacientes con comorbilidades
(cirrosis heptica, hepatitis crnica por virus C, hepatitis
B con indicacin de tratamiento, CVP > 105 copias/ ml,
proporcin de CD4 < 14%, edad > 55 aos, riesgo
cardiovascular elevado y nefropata por VIH) (nivel B).
4. En parejas serodiscordantes debe valorarse
cuidadosamente el inicio de TAR para disminuir la
transmisin por va heterosexual (nivel B). En ningn
caso ello debe suponer la abstencin de otras medidas
para impedir la transmisin del VIH.
A pesar de las consideraciones previas, el inicio del
TAR debe valorarse siempre individualmente. Antes de
tomar la decisin de iniciarlo deben confirmarse la cifra
de CD4 y de CVP. Adems, debe prepararse al
paciente, ofertando las distintas opciones, adaptando el
esquema teraputico al estilo de vida, comorbilidades,
posibles interacciones y valorando el riesgo de mala
adherencia.

Ensayo clnico, controlado, randomizado, abierto,
llevado a cabo en Hait
Se randomizaron 816 pacientes mayores de 18 aos a
iniciar TARGA (AZT+3TC+EFV) entre 200 y 350
clulas CD4/L o diferirlo hasta 2 semanas despus
del diagnstico clnico de SIDA o que su recuento de
CD4 descendiera por debajo de 200 clulas/mL.
El estudio fue interrumpido prematuramente tras un
anlisis interino planeado que mostr 6 muertes en el
grupo de 200 a 350 clulas/mm3 frente a 23 en el
grupo de pacientes que difirieron TARV.
Adems, en el grupo de TARV diferido se
diagnosticaron el doble de casos de tuberculosis que
en el grupo de inicio inmediato

VIH Y ENFERM. NO RELACIONADAS
En la cohorte D.A.D.199 y en la cohorte CASCADE200 se
ha comunicado que existe una disminucin progresiva del
riesgo de enfermedades no relacionadas con el VIH a
medida que aumenta la cifra de clulas CD4. Los pacientes
que presentan el menor riesgo de desarrollar enfermedades
no definitorias de sida son los que mantienen una cifra de
CD4 mayor de 350 clulas/uL.
En el ensayo clnico SMART201 la incidencia de
enfermedades no relacionadas con sida aument
significativamente en el grupo de pacientes que
suspendieron el TAR cuando la cifra de CD4 era menor de
350 clulas/ uL. En el ensayo clnico FIRST198 tambin se
ha puesto de manifiesto una relacin entre la cifra de CD4 y
el riesgo de desarrollar enfermedades no definitorias de sida,
siendo menor el riesgo en aquellos pacientes que
mantuvieron la cifra de CD4 por encima de 350 clulas/uL.
En resumen, los estudios de
cohorte sugieren que existe
una relacin progresiva entre
la inmunodeficiencia y el
riesgo de muerte y/o
progresin a SIDA.
Comparado con los pacientes
que empiezan el TAR con
>350 CD4/mL, el riesgo de
progresin est claramente
aumentado en los pacientes
que lo empiezan por debajo.

Los estudios de cohorte sugieren que el riesgo de progresin est
aumentado por encima de los 5560 aos de edad.
OBJECTIVE: The study objective was to analyze the characteristics and the
response to therapy in the eldest of the older adults living with human
immunodeficiency virus.
METHODS: The study included a cohort of patients with human immunodeficiency
virus aged 55 years or more on initiating highly active antiretroviral therapy
(HAART). Immunologic and virologic response, morbidity, and mortality were
assessed. Patients were categorized as aged less than 65 years and 65 years or more.
RESULTS: A total of 112 patients were included (82 patients aged<65 years and 30
patients aged> or =65 years). There were no differences between the age groups in
baseline characteristics, survival, and virologic response. There was a trend toward
better adherence and a lower CD4+ cell increase after HAART in the older group.
CONCLUSION: A relationship was found between lower CD4+ cell increase
after HAART and advanced age. We found the best adherence to treatment in
the eldest of the older adults, and this has been shown to be the only protective
independent factor related to virologic failure.

HIV Transmission Risk in Heterosexual
Serodiscordant Couples (3.381 parejas)
Background: Limited observational studies indicate that ART is associated with
decreased risk of HIV transmission, likely through reduced plasma and genital HIV
levels.
Couples were followed for up to 24 months, with HIV testing of uninfected partners
every 3 months. Seroconversions were assessed for linkage within the couple by
HIV sequencing.
Conclusions: This large prospective study demonstrates that ART use is associated
with substantially lower risk for HIV transmission among heterosexual, African,
HIV serodiscordant couples, where the HIV-infected partner did not meet national
criteria for ART initiation at enrollment. A single transmission occurred within 1
month after ART initiation.
