This document discusses unsafe acts and unsafe conditions in the workplace. Unsafe acts are human errors that violate safety rules, like not wearing protective equipment. Unsafe conditions are hazards resulting from human error, like unguarded machinery. The document states that 80-90% of accidents are caused by unsafe acts rather than unsafe conditions. Examples of unsafe acts include operating equipment without permission or warning others. Examples of unsafe conditions include lack of machine guarding or fire and explosion risks from faulty equipment.

Unsafe Acts • Unsafe acts are performed any time an employee fails to abide by safety rules and protocols. These actions include fighting, horseplay, or performing a job without the necessary safety equipment. • Unsafe acts are extremely dangerous, especially in industries that require workers to handle heavy equipment or hazardous materials on a regular basis. In certain cases, these acts can even result in permanent injury or death. • Employees who engage in unsafe acts may be subject to formal disciplinary action or termination, due to the risk involved. Example of Unsafe Act • Operating any Equipment without proper authority. • Failure to warning. • Operating at unsafe Speed. • Failure to comply PPE’s. • Using hands instead of Tools and Equipment. • Unsafe loading, placing or stacking. • Unsafe position / posture. • Working on moving Equipment. • Wearing loose clothes while working on running Machine. • Working at height without fall prevention. Unsafe Conditions • Unsafe conditions are hazards that have the potential to cause injury or death to an employee. • Some of these hazards include erroneous safety procedures, malfunctioning equipment or tools, or failure to utilize necessary safety equipment such as goggles and masks. • Unsafe conditions can be found in a variety of workplaces, but they pose a special hazard to workers in industrial, manufacturing, or manual labor positions. At the time of hire, companies should provide workers with the information and training they need in order to avoid unsafe conditions. Example of Unsafe Conditions • Lack of guarding on machine. • Crowding workers into one area. • Using the wrong tool for the job Inadequate alarm systems. • Fire and explosions risk. • Faulty Equipment. • Waste going into the wrong waste stream. • Failure of any equipment or associated systems that are in place to protect health and safety. • Failure identified by statutory testing. • Failure of building facilities e.g lift lack of FW support room temperature problems etc. Examples: Condition – Exit Act – Individual set up access scaffolding unsafely, no blocked/obstructed. PPE, no fall protection. Act – Act – Condition – Open access Individual standing hatch (fall hazard). on ladders he set up unsafely. Thank You
