requirements to ensure that all jobs are carried out safely at the Lotte-PPTA
site and Raw Water Pumping Station.

Definition: Where the work is to be executed by an individual, or group of individuals not under the
direct supervision of the person who has operating control of the asset.

Conditions for PTW Exemption
Simple routine tasks of short duration that the Production Manager,
Engineering Manager and HSE & Technical Training Manager have agreed,
do not require a PTW and subsequently authorized by the GM
Manufacturing .
The authority that work can start rests with the Issuing Authority who retains the
authority to stop work at any time should he consider it necessary to do so.

Responsibilities
The PTW Issuing and Accepting Authorities shall have face to face
discussion of the work to be carried out to ensure they have an equal
understanding of the
A joint site visit involving PTW Issuer and Acceptor shall be carried out and

HOT WORK PERMIT
Definition: Hot Work is defined as those work activities, which use or may produce flames, sparks or
other forms of heat energy, which could act as a source of ignition e.g.: a) Arc welding b) Equipment with
naked flames such as burning torches, blow lamps, bitumen boilers or space heating etc.

5.2.3 Vessel Entry Permit (VEP)
Vessel Entry Permit (VEP) Issuing Authority
Definition: A confined space is any item of plant equipment or civil structure enclosing a space at least large enough for
entry and where there is liable to be present, dangerous fumes or a deficiency in oxygen. Holes in the ground shall be
included if more than one meter deep and presenting a significant hazard depending on the nature of the site. The pipe
trench area of Utilities Plant is exempted from this where oxygen deficiency and dangerous fumes are not liable to be
