AGENCY PROJECT MEAGHAN

MCDONALD
• Transportation is being used for patients being
discharged from Monarch or the psych unit who might
TRANSPORTATION
have dual diagnosis and want to treat the substance
abuse issues.
• A bus running Mon, Wed, Fri beginning at 8 am.
• For patients referred for suboxone treatment
WHAT? • A consecutive route that patients expect therefor they
are confident of their own attendance of the
meetings/treatment.
• The bus picks up at Monarch at 8 am Mon, Wed, Fri and
BUS returns around 12-12:30.
• Strictly for Suboxone treatment
HOW?

• SRMC is already partnered with SEATS (transportation service)

• Present SEATS with the proposal, and instruct on the route, pick-up, drop-off
• Deciding on whether we use Medicaid or non-Medicaid
• Also emailed Southeastern health foundation who writes grants with the proposal
PEER SUPPORT SPECIALIST

• Play a huge part in the patients next step in the recovery process
• They help facilitate which patients would be attending these suboxone clinics, therefor
who would be on the bus.
• They have helped around 50 patients
SECONDARY VEHICLE TRANSPORTATION

• SE Health received a $137,500 transportation grant in October

• This grant allows volunteers to use their own vehicles to transport patients to follow-up
appointments, and this includes behavioral health.
• To qualify:
• Adult
• High-risk patients
• Financially disadvantaged
• Patients must also be identified as frequent users of SRMC and emergency department
CONT.

• This grant pilots an expansion of a previous program started in 2016, U-Care

Connections.
• This program partners with local congregations to improve health in communities.
• Where are first volunteers for transport were called
GRANT GOALS

• Short-range goals
• Patients receive transportation for follow-up appointments
• Pharmacy trips after being discharged

• Long-range goals
• Organizing a software for coordinating transports
• Expanding mileage reimbursements to grow program from 4 to 12 drivers
• The idea of transportation for patients who are unable to ULTIMATELY
get it, allows patients to be more accountable. Not only are
they accountable, the follow-up care they need is vital to
better health therefor they will be guaranteed a chance to
get it.
• We know that following up on appointments can be difficult
when one does not have the transportation to get there, but
it is key in recovery as well as curing those who are sick that
they follow-up.
