PHOSPHATE BINDERS AS A CAUSE OF HYPOTHYROIDISM IN
DIALYSIS PATIENTS :
PRACTICAL INDICATIONS FROM REVIEW OF LITERATURE
ABSTRACT
Although fatigue is common in dialysis
patients, polypharmacy is seldom listed
among its cause.

In this report, we describe a dialysis


patient who developed severe fatigue due
to pharmacological interaction between
two commonly prescribed drugs,
phosphate binders and levothyroxine

• Patient with ESRD exhibit various changes in
thyroid function.
• Levothyroxine is commonly used to treat
ESRD patients while there are many foods and
other drugs interfere its absorpsion.
• This study review the literature and draw
conclusion regarding the interferences between
levothyroxine and phosphate binder
CASE PRESENTATION
• A 65 yo woman, on dialysis for 17
yrs, suffer from exhausted and she
thought it was because she was
taking too many drugs.
• The patients is inteligent,
independent, acculturated woman
who lives alone in French
countryside
Her medical history is complex : she was treated with lithium for about 20 yrs (from 20
yrs to 40 yrs). This was discontinued after she developed CKD; in recent years, under
treatment with valproic acid and lamotrigne, her phycophysical balance has been
good.
She underwent total
She started smoking when she
was 19 yo (30 cigaretes a day) thyroidectomy for papillary
and developed a smoke-related ca at age 41 yo, and started
chronic obstructive pulmonary levothyroxine therapy
disease. She was obese in early afterwards. Due to a
adulthood, arterial progressive worsening of
hypertension was diagnosed
the kidney function, she
when she was 30 yo and DMT2
at age 32 yo, treted using oral stared HD at age 50 yo. Her
hypoglicaemic drugs, but she kidney disease was
eventually lost about 20 kg, propbably multifactorial
making it possible for (HT, DM,Obesity,Heavy
antidiabetic drugs to be Smoking, Lithium therapy)
discontinued.
