USE OF SUCTION IN PATIENTS WITH MECHANICAL


VENTILATORS IN INTENSIVE CARE UNIT (ICU) IN
SANGLAH HOSPITAL DENPASAR BALI

PENDAHULUAN
One of the interventions carried out by nurses in the intensive room in a critical state is the
implementation of suction of the respiratory tract (suction), especially in patients who are
attached to a breathing apparatus or ventilator. One of the minimum standard equipment in the
Intensive Care Unit (ICU) includes mechanical ventilation which serves to help patients breathe
through Endotracheal Tube (ETT) or trachealostomy. Patients who use the mechanical ventilator
get sedatives, strong analgesics and muscle relaxants. This makes the patient unable to discharge
secretions spontaneously so that the patient can be at risk of getting pneumonia.
METHODOLOGY Tradition of Excellence
In making this literature review, we use several journals. The
journals that we use to become literature are published in the
last five years, from 2014-2019. The keywords used to search
are suctioning, endotracheal, vital sign, cardiacrespiratory,
mechanical ventilation. The inclusion criteria used in selecting a
journal are using original articles and reviews of international
journals. The journals used in this literature review are 6
journals

1. To evaluate the effect of applying endotracheal suction guidelines to patient's
ventilated mechanical cardiorespiratory parameters
2. To compare the effects of the closed suction method and open suction on blood
pressure, arterial pressure, heart rate, and oxygen saturation in patients under
mechanical ventilation
3. To investigate the effects of minimally invasive ETS (MIETS) versus routine ETS
(RETS) on physiological indices in intubated adult patients
4. To compare the effects of the deep suction method and superficial suction on
respiratory rate (RR) and oxygen saturation in patients admitted to the ICU
5. To determine the effect of endotracheal suction methods with and without normal
saline on hemodynamic and respiratory patients with mechanical ventilation
6. To compare the effectiveness of two negative levels of suction pressure from 100 to
200 mmHg for removing airway secretions by applying the CSS system with hyper
oxygenation in adult patients

DISCUSSION

The use of mechanical ventilators and tracheal intubation is a


form to maintain the condition and stability of patients
treated in the Intensive Care Unit (ICU). The condition of
patients with endotracheal intubation and mechanical
ventilators causes depression of cough reflexes and loss of
muco-ciliary function, and ultimately causes accumulation of
secretions and disrupts the airway. One common way to
overcome the condition of the ineffectiveness of airway
cleaning is to take suction or sucking action.

DISCUSSION

n several studies, suctioning caused a change in the patient's


condition, including changes in RR, oxygen saturation, vital sign,
hemodynamics and reduction in the patient's lung volume. From
the literature review conducted, it is known that RR changes,
oxygen saturation, vital sign, hemodynamics and reduction in
the patient's lung volume can be minimized by not doing
suction for more than 10 to 15 seconds and with suction that is
not too deep.
