HOME VISIT

Definition : a home visit is a professional face to


face contact made by a nurse to the patient or
the family to provide necessary health care
activities and to further attain an objective of
the agency. It is made to the client or to a
responsible member of the family
Factors to be Considered in Determining the Frequency of Home Visit

1. the physical, psychological and educational needs of the individual and


family

2. the acceptance of the family for the services offered, the willingness and
interest to cooperate

3. take into account other health agencies and the number of health
personnel already involved in the care of a specific family

4. the policy of a given agency and the emphasis placed on a given health
program

5. a careful evaluation of past service given to a family and how this family
made use of such nursing services

6. the ability of the patient and his/ her family to recognize their own needs,
their knowledge of available resources on their accord.
Bag Technique
Definition:

Bag technique – a tool making use of a public health bag through


which the nurse during his/ her home visit, can perform
nursing procedures with ease and deftness, saving time and
effort with the end in view of rendering effective nursing care

Public Health bag – is an essential and indispensable equipment


of the public health nurse which he/ she has to carry along
when he/ she goes out home visiting. It contains basic
medications and articles which are necessary for giving care

Rationale : to render effective nursing care to clients and/ or


members of the family during home visit
Special Considerations in the Use of the Bag

• the bag should contain all necessary articles, supplies and


equipment which may be used to answer emergency needs

• the bag and its contents should be cleaned as often as possible,


supplies replaced and ready for use at any time

• the bag and its contents should be well-protected from contact with
any article in the home of the patients. Consider the bag and its
contents clean and/or sterile while any article belonging to the
patient as dirty and contaminated

• the arrangement of the contents of the bag should be the one most
convenient for the use to facilitate efficiency and avoid confusion

• Hand washing is done as frequently as the situation calls for help


in minimizing or avoiding contamination of the bag and its contents

• the bag when used for a communicable case should be thoroughly


cleaned and disinfected before keeping and re-using
NURSING CARE IN THE HOME

• giving to the individual patient the


nursing care required by his/ her
specific illness or trauma to help
him/her reach a level of functioning
at which he/she can maintain
himself/ herself, or die peacefully in
dignity
• the performance of nursing care should
recognize dangers in the patient’s over-
prolonged acceptance of support and
comfort
• nursing care is a good opportunity for
detecting abnormal signs and symptoms,
observing patient’s attitude towards care
given and the progress exhibited by the
patient
ISOLATION TECHNIQUES IN THE HOME
• All articles used by the patient should not be mixed with the articles used by
the rest of the members of the household

• frequent washing and airing of beddings and other articles and disinfection of
room are imperative. Abundant use of soap, water, sunlight and some
chemical disinfectants is necessary

• the one caring for the sick member should be provided with a protected gown
that should be used only within the room of the sick

• all discharges, especially from the nose and throat of a communicable disease
patient, should be carefully discarded

• articles soiled with discharges should first be boiled in water 30 minutes before
laundering. Those could be burned, should be burned
INTRAVENOUS THERAPY

Definition: insertion of a needle or catheter /


cannula into a vein, based on physician’s
written prescription. The needle or catheter/
cannula is attached to a sterile tubing and a
fluid container to provide medication and
fluids
• nurse qualified to perform IV insertion are those who
are duly accredited as I.V. nurse by the Association of
Nursing Service Administration of the Philippines
(ANSAP)
BASIS OF PRACTICE
• Legal therapeutic prescription of a licensed physician

• thorough knowledge of the vascular system,

• interrelatedness of the body system with proficiency in the skill of the I.V. Therapy

• recognition of holistic approach to patient care

• collaboration with members of the health care team

• networking and linkages with the external environment

• the utilization of the nursing process through

• assessment – collection of pertinent data to arrive at a nursing diagnosis.


• planning – formula of nursing care plan relevant to I.V therapy.

objectives.
Environmental Sanitation
- study of all factors in man’s physical environment, which may exercise a
deleterious effect on his health, well-being and survival.

• reuse and garbage disposal
• excreta disposal
• insect vector and rodent control
• housing
• air pollution
• Noise pollution
• radiological protection
• institutional sanitation
• steam pollution
Environmental Health Service (EHS)
- responsible for the promotion of
healthy environmental conditions and
prevention of environmental related
diseases through appropriate sanitation
strategies.
Strategies include:

• water quality surveillance


• evaluation of food establishments
• proper solid and liquid waste management
• sanitation of public places
• sanitation management of disaster areas
• impact assessment of environmentally critical
projects
• enforcement of sanitation laws, rules, regulations
and standards
EHS – also responsible for the conceptualization
of new programs/ projects to contend with
emerging environmentally related health
problems such as:

– Water for Life

– Hospital Waste Management

– Urban Health and National Projects

– Pasig River Rehabilitation Project


MAJOR ENVIRONMENTAL SANITATION
PROGRAMS:

Level I ( Point Source)
• a protected well or a developed spring with an outlet but without a
distribution system
• must not be more than 250 meters from the farthest user
Level II (Communal Faucet System or Stand Posts)
• composed of a source, a reservoir, a piped distribution network and
communal faucets
• located at not more than 25 meters from the farthest house
• one faucet per 4 to 6 households
Level III ( Waterwork System or Individual House Connections)
• A system with a source, a reservoir, a piped distributor network and
household taps.
• Unapproved Type of water facility
• Water coming form doubtful sources such as open dug wells,
unimproved springs, wells that needs priming unless treated through
proper container disinfection
• All households shall be provided with safe and adequate water
supply.
Water Quality and Monitoring
Surveillance
• every municipality through its Rural Health Units
must formulate an operational plan for quality and
monitoring surveillance every year
• the examination of drinking water shall be
performed only in private or government
laboratories duly accredited by the Department of
Health
• Certification of potability of an existing water source
is issued by the Secretary of Health
Disinfection of water supply sources is required on
the following:

• newly constructed water supply facilities


• water supply facility that has been repaired/
improved
• water supply sources found to be positive
bacteriologically by laboratory analysis
• container disinfection of drinking water collected
from a water facility that is subject to
recontamination like open dug wells, unimproved
springs and surface water
Proper Excreta and Sewage Disposal Program
Approved types of toilet facilities

Level 1
• non-water carriage toilet facility
• no water is necessary to wash the waste into the
receiving space
• pit latrines, reed odorless earth closet
• toilet facilities requiring small amount of water to
wash the waste into the receiving space
• pour flush toilet, aqua privies
Level 2

• on site toilet facilities of the water carriage type


with water-sealed and flush type with septic vault/
tank disposal facilities

Level 3

• water carriage types of toilet facilities connected to


septic tanks and/or to sewerage system to
treatment plant
Food Sanitation Program

Food establishments shall be appraised as to the following sanitary


conditions:

• inspection/ approval of all food sources, containers, transport


vehicles

• compliance to sanitary permit requirements for all Food


establishments

• provision of updated Health Certificate for food handlers, cook


and cook helpers which include monitoring as to presence of
intestinal parasites (ascaris, amoeba, etc.) and bacterial
infectious (typhoid, cholera, dysentery, salmonella infections and
others)
• destruction or banning of food unfit for human consumption
• training of food handlers and operators on food sanitation
• food establishments shall be rated and classified as follows

– Class A – Excellent
– Class B – Very Satisfactory
– Class C – Satisfactory

• Ambulant food vendors shall comply with the requirements


as to the issuance of health certificate which also include
monitoring the presence of intestinal parasite and bacterial
infection
• Household food sanitation are to be promoted and
monitored and food hygiene education to be intensified
through health education and provision of IEC materials
Hospital Waste Management Program
– All newly constructed/ authorized and existing
government and private hospitals shall prepare and
implement a Hospital Waste Management (HWM)
Program as a requirement for registration/ renewal for
licenses
– The use of appropriate technology and indigenous
materials for HWM system shall be adopted
– Training of all hospital personnel involved in waste
management shall be an essential part of hospital training
program
- Public information campaign on health and environmental
hazard arising from mismanagement of hospital shall be
the responsibility of hospital administration

- DOH hospital Waste Management guidelines/policies shall


be guided by existing legislative health and environmental
protection laws/ policies on waste management

- Local ordinances regarding the collection and disposal


techniques, especially incinerators, shall be
institutionalized
Nursing Responsibilities and Activities
Health Education

• development and dissemination of IEC materials and messages, their


utilization and distribution
• providing individual and group counseling
• use of other mass media support and IEC materials including holding of
community assemblies and bench conferences
• Actively participate in the training component of the service and attend
training/workshops related to environmental health
• assists in the de-worming activities for the school children and targeted
groups

• effectively and efficiently coordinate programs/ projects/ activities with


other government an non-government agencies

• acts as an advocate or facilitator to families in the community in


matters of program/ projects/ activities on environmental health in
coordination with other members of the Rural Health Unit (RHU)
especially the Rural Sanitary Inspectors
• Actively participate in environmental sanitation campaigns and projects
in the community
• Be a role model for others in the community to emulate in terms of
cleanliness in the home and surrounding

FIELD HEALTH SERVICES AND
INFORMATION SYSTEM (FHSIS)
Objectives:

• to provide summary data on health service delivery and


selected program accomplished indicators at the barangay,
municipality/city, district, provincial, regional and national
levels

• to provide data which when combined with data from other


sources, can be used for program monitoring and evaluation
purposes

• to provide a standardized, facility level data base which can


be accessed for more in-depth studies
• to ensure that the data reported to the FHSIS are useful and
accurate and are disseminated in a timely and easy to use
fashion

• to minimize the recording and reporting burden at the


service delivery level in order to allow more time for
patient care and promotive activities

- document, form, or piece of paper upon which recorded the


presenting symptoms or complaints of the patient on
consultation and the diagnosis if (available), treatment and date

Qualifications : Bachelor of Science in Nursing, Registered Nurse in

Roles
• Planner or Programmer
– identifies needs, priorities and problems of individuals, families and

– formulated nursing component of health plans. In doctor less area,
she/he is responsible for the formulation of the municipal health plan

Qualifications : Graduate of an accredited School of Midwifery and passed the
Midwifery Board Examination

DUTIES AND RESPONSIBILITES

• Under the general supervision of the Public Health Nurse (PHN), the Rural Health Midwife
shall perform the following functions:

• Works with the PHN in planning and evaluating health services at the barangay level:

• Participates in determining health needs of individuals, families and the community

• Assists in planning and organizing the clinic in his/her barangay health station

• Prepares monthly schedule of activities in coordination with the physician, nurse, sanitary
inspector and other health workers
• Plans for activities in the clinic Barangay Health Stations
(BHS), follow-up in homes and field visit in the community
– Participates in the periodic evaluation of health services in the
barangay

• Provides midwifery services in the barangay

• Gives direct care to normal childbearing women during


pregnancy until the end of pueperium as well as to normal
newborn infants

• Refers to physician and/or nurse and appropriate agencies


pregnant and post-partum women and newborn infants
with suspected abnormalities and problem
• Carries out medical nursing functions as authorized by the
Department of Health

• Conduct clinic which includes:

• Obtaining clinical history


• Performing simple routine physical and laboratory
examinations

• Administering emergency and therapeutic (medical and


nursing) measures based on the standing orders

• refers cases needing further diagnostic and management by


nurse and/or physician and other agencies

Functions and Responsibilities

• Assesses needs for community health nursing services at the


provincial/ city level

• Participates in the formulation of the provincial/ city health


plan with the medical, technical and administrative staff

• Develops nursing components of the provincial/ city health

• Organizes and directs implementation of community health


nursing services and other health program
• Supervises, monitors and evaluates implementation of health programs/ projects

• Interprets policies, guidelines and standard operating procedures to nursing and


midwifery staff of the province/ city

• Assesses training needs and plans for staff development program for nursing and
midwifery staff

• Conducts training for nursing and midwifery staff and participates in the training
and evaluation of other health workers of the province/ city

• Establishes linkages with government and non-government organizations for


effective health care delivery in Rural Health Units (RHUs)/ Health Centers

• Implements a continuous training program for nurse and

job training for nurses and midwives conducted by the
Department
• Attends and participates actively in workshops,

years of which are in training or nursing education

Functions and Responsibilities

• Assesses training needs of nursing and midwifery staff, both hospital and
public health ( Supervising Public Health Nurses, Midwife Supervisor)

• Designs and organizes training program for nursing and midwifery staff
( hospital and public health) and participates in the planning and
organizing training program for other workers

• Develops and prepares training materials/ devices for training


• Conducts, monitors and evaluates training programs for
nursing and midwifery staff and participates in the training of
the health workers and midwifery staff

• Supervises, monitors and evaluates the performance of the


Nurse instructor both in hospital and public health

• Analyzes and interprets reports of Nurse Instructors

Functions and Responsibilities

• Assesses needs for Community Health Nursing Services as part of the total
health program

• participates in the overall planning of the city health department program


with the medical, technical, administrative and other nursing staff

• plans, organizes and directs the implementation of community health


nursing services with the assistant chief nurse and district nurse
supervisors

• supervises and coordinates the activities of the Assistant Chief Nurse,


Nurse Supervisors and Nursing Training Staff

• Evaluates the quality and effectively of nursing services in relation to the


total health program
• coordinates community health nursing services with other health
and socio-civic programs/ agencies. (Government and Non-
Government)

• acts as Consultant to the City Health Officer and nursing staff


on nursing and related matters

• plans for and motivates, nursing staff to pursue continuing


education programs for professional growth and development.
Keeps himself/ herself abreast on trends and developments on
nursing and related fields

• participates in the recruitment selection of nursing personnel


and makes proper recommendations. Recommends deserving and
qualified nursing personnel for promotion
• initiates, motivates and participates in the conduct of studies/
researches to improve effectiveness of nursing services

• attends conferences, seminars and updates on nursing


community, heath and related fields

• participates in community outreach programs as needed

• provides leadership and encourages development of


leadership abilities among nursing personnel

• serves as resource speaker/ reactor in staff development and


other training activities

• performs other functions assigned from time to time


ASSISTANT CHIEF NURSE IN SELECTED CITY HEALTH
DEPARTMENTS

Qualifications : Bachelor of Science in Nursing,


Registered Nurse in the Philippines, Master’s Degree
in Nursing, Major in Community Health Nursing or
Master’s Degree in Public Health/ Nursing
Administration with at least five years experience in
community health nursing, two years of which have
been in supervisory position, with demonstrated
leadership ability
Functions and Responsibilities

• Participates with the chief nurse in the assessment of nursing needs

• participates in planning, organizing and directing the implementation of


Community Health Nursing Services

• Supervises, coordinates and evaluates the performance of the District


Nurse Supervisors and assists them in the evaluation of community health
nurses

• participates in the recruitment and selection of nursing personnel and in


the orientation of new community health nurses

• analyzes potentials of Nurse Supervisors for professional growth and


recommends deserving nurses for study grants
• participates in the planning and conduct of staff development programs
for nurses

• work with professional organizations and other socio- civic groups (GOs
and NGOs) to improve nursing services

• acts resource person in seminars and meetings on nursing, community


development and related matters

• participate/ conducts studies/ researches in community health nursing


to improve service

• performs the functions of the Chief Nurse in her/his absence

• attends meetings, seminars, workshops on nursing and related fields

OCCUPATIONAL HEALTH NURSE
Qualifications : Registered Nurse, Bachelor of Science in Nursing
preferably with training/ units in Occupational Health Nursing
Functions and Responsibilities:

• Assists or participates in developing an adequate health program for


workers and laborers, which should include sound health education
activities

• encourages periodic physical examination and provides guidance or


direction in obtaining his/ her service

• cooperates with occupational medical programs in the presentation of


accidents and in the promotion of good working atmosphere and
relationships in the place of work

• determines resources for rehabilitation of the laborers and employees


whenever indicated
• assists the under-served small scale industries to maintain
health and safety of the workers

• interprets to management the Provision of Rep. Act 1054 in


which the employer shall maintain and keep

• inspects commercial and industrial establishments to check


the sanitary conditions and to detect the presence of
industrial occupational hazards and to report same to the
Chief of the Service

• gives health teaching on how to detect and prevent risks


and occupational related diseases

• guides and helps an industrial or nurse with the view of


improving the services in occupational health
– understanding of the nature of risks and how the workers
are exposed

– meeting emergency cases promptly and adequately

– applying up-to-date techniques and procedures and


translating to employees the safety policies of the
company

– assessing, developing and implementing control measures


among employees and their families

– counseling to help employees meet their problems

– keeping and maintaining good and simple records


– utilization of the community resources for the benefit of the
employees

• prepares and submits daily, monthly, and annual


reports of accomplishment

• keeps himself/ herself abreast of modern trends in


nursing through active participation in professional

OCCUPATIONAL HEALTH NURSE SUPERVISOR

Functions

• Participates in the formulation of policies,


development of plans/ programs on matters pertaining
to the prevention and control of occupational health
diseases

• assists in the preparation of standards and guidelines


for information, education and communication

• participates in the development of training programs


on occupational health and safety
• assists in the identification of training needs of lower level
health personnel on occupational health diseases

• performs the two key elements of Occupational Health


Programs such as:

health protection – comprising the health risk management at


work which is the health hazard identifications, health risk
assessment, its control measures, health surveillance,
information, instruction and training, monitoring and record
keeping and
health promotion comprising the assessment of health risk
associated with the living environment and lifestyle, employee
information and assistance program
