CODE ON DISCIPLINE
Code of discipline entails a process of growth, progress and development for all students. It is governed by the domains of learning that serve as pillar for motivation and enhancement of knowledge, skills, attitudes and perspectives. Discipline provides cynosure of learning within societal norms. The learning experiences gained through clinical and field experience yield and provide strong rudiment of attainable goal and aspiration of becoming well bred, skilled, competent nurses. It is within the College of Nursing discretion to formulate standardized operating procedures with which help the students and faculty to implement policies to set for the higher level of competency. In lieu with this, the College of Nursing provides related learning experience in relation to the mission and vision of the University of Makati wherein modification and ossification of values is of great importance.

The College of Nursing formulated and standardized disciplinary action for the purpose of correcting rather than punishing a wayward student. In attempting to correct unacceptable students behavior, the following psychological principles should be applied: Following rule infraction by students, discipline should be administered promptly, privately, thoughtfully and consistently. Discipline should be progressive in nature and should be preceded by counseling. Caution should be observed in instituting disciplinary procedures.

VARIOUS OFFENCES & DISCIPLINARY ACTIONS


1. ATTENDANCE
Students are expected to report in every scheduled tour of duty in a given area of the specified hospital. Students who absent himself needs to present to Clinical Instructor the next scheduled duty an excuse letter duly signed by the College Secretary including medical certificate (UMAK clinic or Duly Recognized Physician General Practitioner). Failure to present an excuse letter duly signed by the College Secretary is considered absent unexcused

DISCIPLINARY ACTION:
Excused Absence =

(1 excused absence = 8 hour make-up duty + oral counseling) Unexcused Absence = (1 unexcused absence = 24 hour make-up duty + written warning) More than 2 excused absence = 1 unexcused absence More than 2 unexcused absence = repeat RLE

NOTE: Student who misses 20% of the required number of hours for the subject in the semester shall be dropped from the corresponding class. (See general rules & regulation on attendance UMAKs Student Handbook).

2. TARDINESS
Students are required to report to duty
regularly and punctually. Every tour of duty, the call time for students is 15 minutes before duty starts. A minute after duty started is considered late reporting for duty. 15-minute late with or without valid reason is considered an absence however student is allowed to go on duty. 30-minute late or more with or without valid reason is considered an absence and student is not allowed to go on duty.

DISCIPLINARY ACTION:
With valid reason = excused late Without valid reason / advise = unexcused late 1 late not more than 15 minutes = oral counseling 2 late (combined) not more than 15 minutes = written warning 3 late = 1 absence 4 late = 2 absence 5 late = 3 absence 6 late = 4 absence 7 late & more = repeat rotation

NOTE: Students are required to complete his / her makeup duties in the next semester otherwise incomplete grade will be reflected. Incomplete grade must be removed within the ONE YEAR, otherwise, it

Students are required to wear the proper uniform at
all times during duty hour. PROPER UNIFORM (FEMALE STUDENT)


UMAK I.D. / UMAK Bar Pin UMAK Nursing Cap White Uniform with UMAK patch (ward) Scrub suit with UMAK patch (special area) Plain white shoes ( rubber sole) Nursing Stockings (white) White chemise, panty let and or half-slip Wrist watch with third hand No jewelries and earrings Plain white underwear (no tongs & T-backs) UMAK Bag / Kit (please see attached list) DISCIPLINARY ACTION : Demerit system will be applied accordingly and penalty shall be imposed before the end of the tour of duty and or semester.

5. PROPER GROOMING
MALE STUDENT Clean hair cut (hair must not touch the ears and uniforms collar) Clean shaven Short fingernails, no colored nail polish. Clean earlobes No exposed tattoos Immaculately clean white shoes

FEMALE STUDENT
Pony tailed-hair (hair must not touch the ears and uniforms collar and no colored hair bands ) Wear black hair pins Wear black hair net Short fingernails, no colored nail polish. Clean earlobes No exposed tattoos Immaculately clean white shoes DISCIPLINARY ACTION : Demerit system will be applied accordingly and penalty shall be imposed before the end of the tour of duty and or semester.

TELEPHONE / MOBILE PHONE USAGE Students are not allowed to use, receive calls, using the hospital phone and send / receive SMS through personal mobile phone while on duty hours. In case of emergency, approval from Clinical Instructor must be obtained.

DISCIPLINARY ACTION : Demerit system will be applied accordingly and penalty shall be imposed before the end of the tour of duty and or semester.

7. STUDENTS BEHAVIOR DURING DUTY-HOUR


Students must be courteous and respectful of Clinical Instructors, Hospital personnel, patient and fellow students. Students must receive endorsement properly. No talking while endorsement is on-going. No loitering No littering No gossiping No cheating on exams No seating on patients chair or hospital bed

8. PENALTY / DEMERIT SYSTEM


The following demerit system will apply: 1 demerit = oral counseling 2 demerit = written warning 3 demerit = 1 late 4 demerit = 2 late 5 demerit = 3 late 6 demerit = 4 late 7 demerit = 5 late 8 demerit & more = repeat RLE NOTE : Students will receive demerit points if they violated the rules / policy on tardiness, proper uniform, proper grooming, telephone / mobile phone usage and students behavior during duty-hour.
